Welcome to Allied Surgical Group
Allied Surgical Group is a multi-specialty general surgical practice. There is practically no operation or surgical problem within the realm of general surgery that at least one of our members cannot take care of.
We would like to ask you to take a moment to read through the information below. This will help to familiarize you with our practice and provide us with the information necessary to optimize the care we deliver to you. Please follow the instructions below to streamline your visit and to allow us to get all the information we need in order to provide you with the highest level of care possible.
1. Please complete the Encounter form prior to your visit and bring it in with you. If you have your paperwork ready ahead of time, you only need to be at your appointment 5-10 minutes prior to your scheduled time. Please pay special attention to the section about medications, and provide us with the exact name, dosage and administration schedule of every medicine you take.
2. Most patients have had previous X-rays, lab tests, biopsies or other tests. WE REQUEST THAT YOU BRING ALL THESE TESTS AND WRITTEN REPORTS WITH YOU PERSONALLY. PLEASE DO NOT HAVE THEM MAILED, FAXED OR FORWARDED TO US AHEAD OF TIME BY YOUR PHYSICIANS. For blood work, simply bring copies of the blood tests. For any kind of X-rays, CT scans or other radiographic examination, you need to bring the written report by the radiologist as well as the actual X-rays. You can request these from the radiology facility where they were performed.
If you have had a biopsy at an institution other than Saint Clare's Hospitals, Morristown Memorial Hospital or Somerset Medical Center, you need to bring the actual pathology slides and the written report by the pathologist. When you pick up these X-rays or other test results, please check with the secretaries who provide them to you to make sure that the corresponding written reports are included with the tests. AS MENTIONED ABOVE, PLEASE BRING THESE WITH YOU PERSONALLY ON THE DAY OF YOUR VISIT INSTEAD OF HAVING SOMEBODY FORWARD THEM TO US PRIOR TO YOUR VISIT, EVEN IF YOUR DOCTOR SAYS HE OR SHE WILL DO SO.
3. Bear in mind that we have several different office locations. Prior to your arrival, please be sure you know in which office you will be evaluated.
We understand that going to a doctor's appointment can be a stressful experience, and we hope to make it as comfortable for you as possible. The more information you can provide us about your medical and personal problems, the easier it will be for us to assist you and make the right decisions.
